Admissions Checklist
Welcome. We are so pleased that you are interested in applying to Nazareth Academy. Here at the Academy any young women making satisfactory progress toward the completion of eighth grade, or any high school student in good standing at her present school, is welcome to apply for admission to Nazareth Academy. Please complete each of the steps outlined below in order to be considered for admission.

Submit Application Materials:

Application for Admission to Nazareth Academy.

Copy of a graded essay from a recent English/ History/Social Studies class including teacher's remarks,
comments or notes

Copies of all that apply: Current neuropsychological evaluation, 504 Plan, Individual Education Plan (IEP).

Parent or Guardian Statement (optional)

The candidate also is responsible for forwarding the following forms to the person in the candidate's school who is responsible for preparing the necessary information. Please note that materials must be mailed directly to Nazareth Academy after the first marking period and no later than January 15, 2011.

Principal/Headmaster/Counselor Report Form


Current English Teacher Recommendation Form


Current Mathematics Teacher Recommendation Form


Interview with Director of Admissions (Students applying from 9th grade or higher)


All application materials must be postmarked no later than January 15, 2011.

Testing

Testing is required for all applicants. While the Admissions Office prefers the SSAT for the 9th and 10th grade applicants, the Catholic Seconday School Entrance Exam, which is given at many of the surrounding Catholic schools is also accepted. Please send a copy of you complete exam scores along with your application.

Applicants for the 11th grade, may substitute the PSAT or SAT for the SSAT or the HSPT.


Recommendation Forms


Principal/Headmaster/Counselor Report Form:

A Principal/Headmaster/Counselor Report Form is included with the Nazareth Academy's admissions materials. The top portion of this form should be completed by the candidate, and then given to the person in the candidate's school who is responsible for preparing records. Upon the report's completion, please forward directly from the candidate's school to Nazareth Academy as soon as possible after the first marking period and no later than January 15, 2011.


Current English Teacher Recommendation Form:

This form is included with the Nazareth Academy's admissions materials. Please follow same directions as above.


Current Mathematics Teacher Recommendation Form:

This form is included with the Nazareth Academy's admissions materials. Please follow same directions as above.


School Transcript:

An official copy of mid-year or final semester school transcript must be submitted with the Principal/Headmaster/Counselor Report Form. A parent/guardian must request this from the current school.


Tuition

Tuition for the 2010-11 school year is $12,500.


We encourage every student to apply for tuition assistance. To apply for tuition assistance, parents are asked to complete and submit a tuition assistance form.
